Here are a couple links to help those interested in obtaining a re-entry decal from the U.S. Customs.  The decal is required of all vessels 30 feet and over for re-entry into the United States.

The Decal / Transponder Online Procurement System is an easy way for Commercial Vehicles, Private Aircraft, and Private Vessels (thirty feet and over) to purchase their Annual User Fee online. Commercial Vehicle purchasers also have the ability to request replacement transponders, transfer vehicles into their fleet, and maintain their fleet inventory.  https://dtops.cbp.dhs.gov/

Colin Mack provided a link to a printable version of the application for the decal.
